Our practice is no longer accepting patients or providing treatment. If there is anything we can help you with as you transition to a new provider — medical records, a summary of your treatment, or information to share with your next prescriber — please reach out.
Continuing your care
- If you are on a GLP-1 (semaglutide or tirzepatide) or hormone therapy, please plan for continuity with a new prescriber before your current supply runs out. We are glad to send your records or a treatment summary — just email us.
